New Patient Registration

How do I register as a patient?

Patients wishing to register must complete a new patient registration form (GMS1) available from reception and you will then be invited to participate in a new patient consultation. Please note that we will not be able to issue repeat medication on the day of registration or until such a time that we have received your full medical records from your previous general practitioner.

Please ensure that you have at least one month’s supply of your regular medication from your previous surgery.
